This web site, called RSVME, features a free application that can help you quickly build surveys to send out to groups of people. It integrates directly with Outlook and Outlook Express. Once you build your survey/questionnaire, you simply pick people from your address book to send it to. The recipients receive a link to a web site that has the survey.
